Health Policy Priorities

ONS bolsters cancer care and empowers oncology nurses through strategic policy on both the legislative and regulatory levels. Our primary objectives are to preserve and promote oncology nurses’ practice to the full scope of their ability, improve the quality of cancer care, and advocate for payment models that incorporate the work of oncology nurses and positively impact oncology practice.

These principles guide our policy agenda and our day-to-day advocacy work. 

Patient Access to Quality Cancer Care

The pace of innovation in oncology treatment is rapidly increasing, with rising numbers of novel active substances and faster speeds to launch. ONS is working to ensure nurses are quickly and proactively prepared to care for patients using these emerging treatments and calls on the U.S. Congress and the administration to improve access to cancer screening, safe and affordable cancer drugs, radiotherapy, and surgery.

Cancer Prevention

Oncology nurses play an integral role in disseminating public health education and disease prevention. ONS will work to encourage the U.S. Congress and the administration to ensure federal funding for initiatives like screening, education, early detection, and prevention through the CDC, encompassing efforts to combat tobacco and e-cigarette usage. Additionally, ONS seeks unimpeded insurance coverage to expand treatment access for smoking cessation, aligning with the surgeon general's cessation report, while also addressing how social determinants of health impact access to advancements in cancer prevention and early detection services. ONS also advocates for legislation and enhances access to cancer screening and decreases exposure to known carcinogens.

Cancer Research

ONS advocates for transformative research to advance cancer prevention, diagnosis, and treatment and to achieve the goals of the National Cancer Moonshot Initiative. ONS encourages the U.S. Congress and the administration to endorse precision oncology and genomics for revolutionary cancer care, address outcome disparities, and sustain funding for health disparity programs. Embracing individual variations, ONS supports innovative treatments and patient awareness of clinical trials, striving for diversity among participants and coverage of associated costs. Amplifying research funding at NIH and related institutes, ONS seeks backing for nurse scientists and expands palliative care and symptom management research. The partnership with the FDA's Project Livin' Label bolsters understanding of oncology product approvals.

Nurse Education Title VIII

ONS recommends that the U.S. Congress and the administration enact the Palliative Care and Hospice Education and Training Act (PCHETA), which would improve palliative care and hospice training for nurses and other health professionals and establish programs for advanced practice providers to train and teach in palliative care, home care, hospice, long-term care, and other areas. ONS also urges sustained federal funding for the Title VIII Public Health Service Act Nursing Education.

Teleoncology

The field of oncology rapidly adapted to the 154% increase in clinical care provided remotely following the onset of the COVID-19 pandemic. The shift was facilitated by relaxed guidelines from the Centers for Medicare and Medicaid Services but occurred without formal resources and education for nurses on how to evolve problem-based care over the telephone to full-care visits over video. ONS asks the U.S. Congress and the administration to promote access to virtual care and telehealth by ensuring permanent coverage, expansion of services, and support for technology and resources to advance this mode of health care. ONS also advocates that where teleoncology is being used, limitations of technology use due to socioeconomic class or digital literacy are considered and addressed to not increase disparities.

Oncology Nurse Workforce Safety and Well-Being

Although nurses have been the number one trusted profession for the past 21 years in a row, they encounter workplace violence and abuse at a higher rate than in any other profession in the United States. ONS calls upon the U.S. Congress and administration to uphold and bolster frontline nurses' well-being and support. Essential actions include addressing nursing burnout and workplace violence, acknowledging the correlation between proper nurse staffing and enhanced patient outcomes, prioritizing safe working environments that reduce exposures, and emphasizing culturally competent and safe clinical practices across healthcare settings.

Scope of Practice

For patients to access the most comprehensive, cost-effective care, oncology nurses must be able to practice to their full licensure and training. Throughout the COVID-19 public health emergency, federal and state agencies eased practice restrictions, maximizing access to care during the pandemic. ONS calls on the U.S. Congress and the administration to preserve those flexibilities and promote oncology nurses’ scope of practice, especially regarding new and innovative models of care delivery, within the Medicare, Medicaid, and Veterans Health programs. In addition, ONS urges the U.S. Congress and the administration to ensure reimbursement policy is inclusive of nursing, including abolishing incident-to-billing and allowing advanced practice RNs (APRNs) to bill directly for services provided.

Value of Oncology Nurses

To bolster the oncology nursing workforce, ONS advocates the need for federal policies to accurately acknowledge and encompass the crucial role of oncology nurses in patient care and outcomes. This involves advocating for comprehensive Medicare payments and policies that cover the full spectrum of inpatient and outpatient oncology nursing services, including those by APRNs. ONS supports establishing distinct "specialty designations" for APRNs in oncology, promoting their unique contributions and aiding in research and equitable healthcare enhancements. The organization also pushes for policies enabling oncology nurses to provide primary care, chronic care management, and advance care planning. Additionally, ONS strives to integrate nursing insights into policymaking and backs the appointment of oncology nurses to relevant state and federal panels, committees, commissions, and boards.

Clarification on Regulatory Requirements to Deliver Principal Illness Navigation (PIN) Services
April 3, 2024

ONS and ONCC wrote to CMS Administrator Chiquita Brooks-LaSure asking for clarification on certification requirements for patient navigation services. 

Medicare Drug Shortage Prevention and Mitigation Program
March 29, 2024

ONS sent a letter to the Senate Finance Committee regarding a draft proposal to address critical drug shortages. 

Cancer Community Letter to Admin on Menthol Rule
March 19, 2024

ONS joined other organizations in continuing to urge the Biden Administration to finalize a rule prohibiting the use of menthol flavoring in cigarettes.

Tobacco use is the leading cause of preventable death in the United States, killing more than 480,000 Americans each year, and the scientific evidence on the health risks associated with secondhand smoke is clear, convincing, and overwhelming.

ONS's support for the Lymphedema Treatment Act, H.R.3630, and to thanks you for your leadership in introducing this important legislation. Amending the Medicare statute to enable coverage for prescribed medical compression garments will help millions of Americans with lymphedema access the care they need to manage this chronic disease.

 

Medical Nutrition Therapy Act
May 21, 2021

ONS writes to thank you for introducing S. 1536, the Medical Nutrition Therapy Act. This important legislation would expand Medicare Part B coverage of outpatient medical nutrition therapy services to a number of currently uncovered diseases or conditions, including cancer. The Medical Nutrition Therapy Act would also authorize nurse practitioners, physician assistants, clinical nurse specialists and psychologists to refer their patients for medical nutrition therapy.

ONS thanks you for introducing H.Res.391, supporting the designation of July 21, 2021 as "Glioblastoma Awareness Day.” Glioblastoma is the most common malignant, cancerous brain tumor. Approximately 13,000 new cases of glioblastoma will be diagnosed in the U.S. in 2021, and it is estimated that more than 10,000 people will die from glioblastoma this year. Oncology nurses are the health professionals involved in the administration and monitoring of chemotherapy and managing the associated side effects cancer patients may experience.

ONS joins 316 organizations to express strong concern with the Centers for Medicare & Medicaid Services’ (CMS) March 8, 2016 proposed rule that would implement a new “Medicare Part B Payment Model.” We believe that this type of initiative, implemented without sufficient stakeholder input, will adversely affect the care and treatment of Medicare patients with complex conditions, such as cancer, macular degeneration, hypertension, rheumatoid arthritis, Crohn’s disease and ulcerative colitis, and primary immunodeficiency diseases. We previously sent a letter to Department of Health and Human Services (HHS) Secretary Sylvia Burwell asking her not to move forward with this type of initiative, and we now respectfully request that you ask CMS to withdraw the proposed rule.

We are writing in response to the Request for Comments on the pilot “Clinical Outcome Assessment Compendium” (COA Compendium). ONS applauds the FDA's focus on measures that capture outcomes that "are important to patients" in clinical trials and efforts to foster patient-focused drug development. ONS supports the benefits of a standardized approach to assessment of patient outcomes, for which the compendium would be an asset.

ONS applauds the protections afforded to healthcare providers as outlined in the bill, to include freedom to recommend treatment options that are in alignment with the goals, needs, and values of patients.
